Installed ET again, but have major lag spikes and my ping doesnt go below 142...

Any ideas?

Also, does anyone have my old config? Long shot I know

I am struggling to remember how to bind my mouse wheel so scroll up is syringe and scroll down is pistol.

Check your network for ping issue. Sometimes ISP or datacenter change routes and hence ping gets messed up.

For lag spikes, is it red or green spikes in lagometer? You need to show some lagometer screenshots before we can help you.

Then your ping is jumping around and hence green spikes. Just fix your router / modem if network is not an issue.

Check in below list:
1. Network - If that is an issue contact ISP and if server network is an issue then change server.
2. Router / Modem - Check both if network is fine.
3. Check PC LAN card or wifi car - If 2nd one is not an issue.

Normally, it's always 1.

If u have Nvidia card and lagspikes are like 1ms per 3secs freezes then go to your nvidia control panel-> manage 3d settings-> change these: multi display/mized gpu acceleration you have it as multi display performance mode change it to single display, then threaded optimization off and of course r_primitives 2? or 1? cant remember, hope that helped even a bit. And well you might have punkbusted fucking you up if u did fresh install of ET then just write pb_sleep 500?
