I have that problem too - nVidia drivers I think... live with it until etrpro / nVidia (unlikely) fix it.

Graphiccard? Guess 7950 or 8800.
New drivers are crappy.

yes downgrade drivers(to under 91.xx) or r_texturebits "32"

from etpro-forum:
#984 - workaround for bug in nvidia 91.xx drivers not displaying commandmap icons properly
Known issues
#984 workaround does not work with 16 bit textures. Use r_texturebits 32 to see the icons on the command map.
