Have you also checked your Vsync if its off ? Also, try in_mouse -1 and m_filter 0 and cl_mouseaccel 0. If you have logitech mouse, you have to also disable acceleration through mouseware.

first r_swapinterwal 0 ingame, and take a look into your video drivers for V-Sync.
