fps drops all the time

When i play ET my fps drops every 3-5 seconds
it doesn't matter if i lock it at 76 or 125
does any1 know what could be the problem here?

i already reinstalled ET, tried other vga drivers and used other configs.

I used to have that too. Never found a solution. I just figured it had to do with windows. Since I had a lot of crap on my pc (like windows) which I never deleted or reinstalled for years.
