My old setup i used for 2 years ago :p
Move: Arrows
Crouch: Right Ctrl
Jump: Right Shift
Sprint: Numpad 0
Prone: Numpad 1
Lean Left: Numpad 2
Lean Right: Numpad 3
Reload: Delete
Use: Enter
Changed to this :)
Move: WASD
Crouch: Left Ctrl
Jump: Space
Sprint: Shift
Prone: C and V

both arms + legs totally streched, both hands about 3-5 cm away from each other, the left one turned to the left (to reach the arrows), the other hand turned to the right to have enough space for the mouse.

i think i hit the angles quite good, but the one from right hand - arm is too big
