-1 is removed from ET as its bugged, 1 is the best working possibility in ET.
1 using direct input -1 no direct input
if u have high DPI u should use 1 to avoid negative accel
1= sometimes buggy when moving the mouse very fast
-1 solves this problem
400dpi 3.0 intellimouse, should i use -1 then since its low dpi? or doesnt it matter?
When you use in_mouse -1 mouseinputs will be put into a buffer before being processed. This is used to prevent negative accel caused by the quake engine. in_mouse -1 aka direct input is slower, you will notice it really hard when railing. Its kinda the same as using vsync, also creates inputlag by buffering inputs.

us default in_mouse 1.

You confused the two. It's vice-versa (like lettu wrote).
