If you are using a flatscreen your Hertz wont go higher then 75. If you are using a CRT you'll probably be able to get 100 Hertz (800*600, using my CRT as an example). Make sure your displayrefresh matches your Hertz, you have no idea how many people are using 60 HZ and are actually playing on 60 FPS. So if you are using a flatscreen:
atm i have 75 hertz on my huyndai imagequest L90d+ (i think its a regular flatscreen, im kinda noob at this) and i play with r_displayrefresh 75 and com_maxfps 100
check your graphic card settings... set it as high it's possible..just dont listen these noobs... if you can't get even stable fps.. still set it as high it's possible... just dont change your resolution coz you might get better fps.... just stay what you're using atm....

actually, r_displayrefresh sets the hz. 0 means default and same like desktop
75 Hertz -> r_displayrefresh 75 -> com_maxfps 75
So if you are using a flatscreen:
100 Hertz -> r_displayrefresh 100 -> com_maxfps 100
