question to english speakers

I have received a form to compile for a missing play.com order, at the bottom there are 2 fields to fil in: "signature" and "print-name". So I know what a sigature is, but what about print-name?

basically, type your name into notepad

print it out using a printer (in size 10 comic sans only btw)

and then glue it to the form

Print name is when you write your name out so people can read it properly, signature is how you sign for something to say it's you.

Hmm that was my first guess, but it seemed unlikely since my name is already written 4 times in the form they sent me, in all capital letters

Print name is generally block capitals
